The Friends and Family Test (FFT) in GP practices is a quick and simple way of collecting patient feedback to help us to improve the services we offer you.
This will not replace our annual patient survey
From the 1st of December, if you visit the surgery you will be about whether you would recommend the surgery to your family and friends, if they needed similar treatment or care. You will be given the opportunity to give your feedback by answering a simple question: “How likely are you to recommend our GP practice to friends and family if they needed similar care or treatment?” You will be asked to choose one of six options, ranging from “extremely likely to extremely unlikely”. You will then be invited to tell us: If we could change one thing about your care or treatment to improve your experience, what would it be?
Your response will be anonymous: you will be able to complete the FFT via our website or at the surgery. If you are unable to answer the question a friend or family member is welcome to respond on your behalf.
Please place your completed response in either the prescription box in the front porch or the suggestion box in the ground floor waiting room
The Friends and Family Test results will be published on our website and NHS England will also publish the results; from around the country, monthly.
The information will give the practice invaluable feedback about what you think of the care and the treatment you have received, which along with existing ways of gathering feedback, will help the surgery to make improvements for patients.

Issues raised in comments may be discussed between relevant members of the Practice.
The information is used for quality monitoring purposes, in line with the expectations of the patients submitting
The form does not require personal information. Any personal information transmitted via this form may be anonymised
by the Practice when this is required to ensure compliance with General Data Protection Regulation.
All submissions are collated and sent to NHS England.
This information is kept for as long as the site is active or it is deleted by a site administrator.
